Your Veterinary History
Who is your current or most recent veterinarian? This is a very important part of the application.
Even if it's been a while since you've had a pet, please list a vet that you've used that we can contact to get an idea of your history of veterinary visits.
We want our shelter pets to go to homes where they will be taken for annual vet checkups and vaccinations.
Please include any other family member's name that this information might be listed under at the vet's office, or if it is listed under a married or maiden name.

Note: If your vet is Dr Galbraith of Pulaski, please email us your vet records, because he is extremely busy helping people and pets.
